Chronic Disease

GOAL

Reduce the disease incidence and prevalence in Carroll County in regards to chronic health conditions such as obesity, cancer, heart disease, diabetes, hypertension, and respiratory disease.

OBJECTIVE

• Improve access to care for residents suffering from chronic diseases.
• Reduce the proportion of adults and children considered obese (NHSHIP).
• Decrease number of hospitalizations for respiratory related ailments.
• Decrease number of hospitalizations for diabetes related ailments.
