The role of the Continuum of Care Facilitator is to bridge the components of the Substance Use Disorder (SUD) Continuum of Care in their respective networks, including health promotion, prevention, early identification and intervention, treatment, recovery supports and coordination with primary health and behavioral health care. The role of the COC facilitators is achieved through the following activities:
- Ongoing community organization and relationship building among stakeholders representing the full continuum of care within the region
- Convening, facilitating and attending opportunities for collaboration (meetings, events) with regional SUD experts and service providers
- Providing information and resources (including information such as stigma reduction strategies and skills such as evaluation and grant-writing) in service to improve systems of care and integration of behavioral and medical healthcare
